Office Hours
Monday through Friday, 8:30 a.m. until 5:00 p.m. Closed Saturdays, Sundays and Holidays.
Please bring the following items with you to your appointment.
- Your insurance card or other health coverage information. Some insurance carriers require you to have a referral or primary care physician authorization before treatment is given. Please check with your carrier if you are in doubt.
- If you are having your knee examined, we suggest that you wear walking shorts.
- Any existing X-rays or MRI of the area to be examined. Please check out your actual MRI films or X-rays and bring them with you.
- Medical records from your referring physician or current treating orthopedist that relate to your orthopedic condition.
